Xpress Learning Academy

Rainbow Xpress Learning Academy has partnered with the State-Funded Early Care Program and all applications will be reviewed for eligibility and all families will be ranked on an eligibility criteria ranking system.

Your application will be processed and ranked based on several criteria including child’s age, income, disabilities, and at-risk factors (foster care, homelessness, TANF, etc.). Please note that RXLA does not enroll families on a “first come, first serve” system and the date of your completed application does not guarantee enrollment into our program.

After completing your application, we encourage you to follow up with our staff regarding the status of your child(ren)’s application. If there is any change in your contact information, income, or child’s health needs after submitting your application, please notify us immediately. Thank you for your interest and providing the requested documentation needed to sate your application process.

Documentation Requirements for General Enrollment

Before you child begins care, we must have the following information on file:

- Enrollment Application
- Completed Child Information Card which includes Authorization Pick-up & Emergency Contact Information/Emergency Care Form.
- Signed Parents Right to Know
- Permission for screen time of use of electronics
- Any court orders on custody and visitation arrangements, if applicable

- An IEP, IFSP, or Section 504 plan, if applicable
- Annually Completed Ages and Stages Questionnaire ASQ-3 and ASQ-SE2 that help monitor your child’s growth and development. This can be completed online. If you do not know how to access please talk to the administrative team for direction.
- Transportation permission, if applicable
- Using electronics, if applicable
- Information regarding allergies

- Special dietary needs
- Documentation of any current illnesses or injuries
- Current medications and completed Medication Administration Record
The Following forms must be completed by child’s physician if applicable
- Special Care Plan for a Child with Asthma
- Food/Environment Allergy Action Plan
- Individual Health Care Action Plan
- Special Diet Statement Form

Within 30 days of enrollment, you must provide your child’s health appraisal that includes his or her health history, a physical examination, growth and development, age-appropriate immunizations, and medical information for treatment in cases or emergency, the child’s blood lead level, and recommendations regarding required medication, allergies, restrictions or modifications or the child’s activities, diet, or care, if applicable.

Additional Documentation Requirements for Enrollment into the State Funded Early Care Program

You will need a copy of at least one of the following documents:

- TANF or SNAP Award letter (amount received)
- SSI Award letter (amount received)
- W-2 form or 1040 tax information.
- Two recent pay stubs (within the last 30 days)
- Unemployment Statement (within the last 30 days)

- Parent/Guardian Photo ID

- Current Dental
